Note that melee weapons such as the Kunai and Electric weapons do not proc the healing part even if a killing blow is made. No love for Hitman though!

Bizarrely the description mentions it reflecting a strong work ethic and team play, team play? How are you contributing to the team outside of killing things and not dying?
Hell if your particularly cynical working with the team is counter productive since your team mates are stealing your damage you need for your health regen.
Grinder is broken, I hope I brought that across and it's been like this for a month n. But what could fix it? Grinder is viewed as it's own playstyle now but it's a build based souly on a perk deck giving it free reign to work with any combination of skills.
Even Dodge build have a limiting factor by limiting the concealment they can have to get the best dodge chance and armour limits your mobility.
Grinder needs a substantial downside to either balance it as a new build type or to bring it back down to the other health regen perk deck.
Becasue of the massive gulf between Death Wish and Overkill means that anything balanced for Death wish becomes overpowered on the 4 previous difficulties.
